Other Consular Services

 

With effect from 1 June 2011, the applications for above listed Other Consular Service will be received at Passport and Other Consular Services Application Centre (PCSAC) at the following address on all working days (Monday to Friday) except public holidays from 0830 hrs to 1330 hrs. (Please note that till 31 May 2011, all applications for Other Consular Services would continue to be received in the High Commission of India.)

Processing time and collection

Collection between 1500 hrs and 1700 hrs on 3rd working day from date of submission at the PCSAC.  However in case the passport was not issued in Kuala Lumpur, issue of the Police Clearance Certificate (PCC) may take up to four weeks.

Documentation for Other Consular Services

Applicants are requested to ensure that the application is complete with the supporting documents.  The specific requirement of documents for various consular services is listed below:

Attestation of driving license

  • Two photocopies of the driving license
  • One copy of the passport (first page, last page and current visa page).

Attestation of Affidavit for obtaining child passport in India:

  • Duly signed affidavit and photocopy of the same
  • Passport photocopy.

Attestation of Marriage Certificate/Birth Certificate:

  • Two photocopies of the certificates
  • One copy of the passport(s).
  • Originals need to be presented for scrutiny.
  • Charges: RM 47+ RM 3.50 service fees

Endorsement of spouse name

  • Duly completed application form EAP 2 with one photo and original passport(s)- click here for Form EAP-2
  • Copies of Marriage certificate and passports
  • Charges: RM 47 + RM 3.50 service fees

5.

Change of Address

  • Duly completed application form EAP 2 with one photo and original passport- click here for Form EAP-2
  • Photo copy of the passport
  • Proof of address like, driving license, ration/election card, bank statement etc.
  • Charges: RM 47 + RM 3.50 service fees

6.

Birth Registration

  • Birth certificate from the registrar of birth and deaths in Malaysia
  • Duly completed Form 1- click here for Form 1
  • Marriage certificate
  • Passport photocopies of both the parents
  • Charges: RM 85+ RM 3.50 service fees

Note: Birth registration and application for new passport can be submitted together.

Death Registration

  • Duly completed application form – click here for Form
  • Original Certificate of death along with copy of the same from the Registrar of Births and Deaths
  • Original passport along with a photocopy of the deceased
  • Police Report
  • Post mortem report
  • Embalming Certificate in case the mortal remains are sent to India
  • Consent letter from the family
  • Letter of undertaking from the employer for settlement of insurance, compensation claims, settlement of dues and repatriation of mortal remains of the deceased to native place in case of legal workers. IC copy of the employer as well
  • No charges for registration of death

8.

Police Clearance Certificate

  • Duly completed EAP-2 application form click here for Form EAP-2
  • Copy of passport (first page, last page and current visa page).
  • Original passport
  • Two passport size photos
  • In case the passport was issued in Kuala Lumpur, the certificate will be issued on the same.  In other cases, it might take up to four weeks.
  • Charges: RM 85+ RM 3.50 service fees

9.

Endorsement of ECNR

  • Copies of the education documents
  • Duly Completed application form EAP-2- click here for Form EAP-2
  • Charges: RM 47 + RM 3.50 service fees

10.

Attestation of power of attorney/ legal documents:

  • The original document along with a copy of the same
  • Two latest passport size photos of executant(s)
  • One photocopy of the passport of the executant(s).
  • Two witnesses
  • Indian national need to execute the POA in the presence of consular officer in the Mission.
  • In case of Malaysian Citizen, the document needs to be attested first by the Consular Section, Ministry of Foreign Affairs, Wisma Putra and then the attested document needs to submitted along with one copy and IC copy of the executant.  The documents relating to POA need to be submitted by the executants.
  • Charges: RM 85/-+ RM 3.50 service fees.
  • In case of commercial documents: RM 199/- + RM 3.50 service fees
