Important Information on Consular Services
 
 

 

Applicants are requested to ensure that the document is complete with the supporting photocopies before collecting token and proceeding to the respective counter.  In case the document is found incomplete and is not accompanied by the supporting photocopies, the applicant will be asked to complete the same and to collect a fresh token for taking the service.

Attestation of driving licence

  • Two photocopies of the driving licence
  • One copy of the passport (first page, last page and current visa page). 
  • The originals need to be presented for scrutiny.

Attestation of Affidavit for obtaining child passport in India:

  • Duly signed affidavit and photocopy of the same
  • Passport photocopy (first page, last page and current visa page).

Attestation of Marriage Certificate/Birth Certificate:

  • Two photocopies of the certificates
  • One copy of the passport(s) (first page, last page and current visa page).
  • Originals need to be presented for scrutiny.

4.

Endorsement of spouse name

  • Duly completed application form EAP 2 with one photo and original passport(s)
  • Copies of Marriage certificate and passports

5.

Change of Address

  • Duly completed application form EAP 2 with one photo and original passport
  • Photo copy of the passport (first page, last page and current visa page). 
  • Proof of address like, driving licence, ration/election card, bank statement etc.

6.

Attestation of power of attorney/ legal documents:

  • The original document along with a copy of the same
  • Two latest passport size photos of executant(s)
  • One photocopy of the passport of the executant(s) .
  • Two witnesses (Indian national) need to sign the POA.
  • In case of Malaysian Citizen, the document needs to be attested first by the Consular Section, Ministry of Foreign Affairs, Wisma Putra and then the attested document needs to submitted along with one copy and IC copy of the executant.

Issue of new Passport in lieu of expiring passport:

  • Duly completed EAP-1 application form

         to be submitted with original passport,

  • Copy of the passport (first page, last page and current visa page). 
  • Four recent passport size photographs with white background (Polaroid photos are not acceptable).

Registration of Child birth and issue of fresh passport to the child:

  • Duly Completed birth registration Form 1,
  • Copy of the birth certificate of the baby Photocopies of the parents’ passports and a copy of the marriage certificate. 
  • EAP-1 application form for passport should be signed by both the parents and needs to be submitted with four passport size photos of the baby with white background (Polaroid photos are not acceptable).

Duplicate passport in lieu of lost/damaged passports:

  • Duly completed EAP-1 application form
  • Sworn affidavit,
  • Eight recent passport size photos,
  • 4 copies of duly filled in additional particular form
  • Copy of the lost/damage passport.
  • Police report
  • Levy receipt in original and other supporting documents

Issue of Emergency Certificate

  • Duly completed EAP-1 application
  • Sworn affidavit, four recent photos
  • Police report,  confirmed air ticket,
  • Copy of the passport and other documents in proof of identity

11.

PIO Card

  • Duly completed application form with 4 photos
  • Statutory declaration duly endorsed by the commissioner of oath
  • Photocopies of the Passport/IC/ birth certificate
  • Proof of India-born/ Indian origin, such as Indian passport, Malaysian passport/Certificate of citizenship showing place of birth as India,
  • Originals are required for verification
